In communities where both teams are present, we are careful to respect the other Primary Care providers in the building

• FHP Geriatrics provides primary care to those wishing for our services to do monthly routine visits, actively manage conditions, anticipate and respond to problems, or

• Agevantx consultants will function as an extra layer of support, facilitate specialist referrals, enhance hospital access, implement medical cannabis if indicated, have complex discussions regarding directives, help navigate difficult family dynamics, and end of life discussions.

  • We will not solicit or offer FHP Geriatrics as a primary care alternative.  We have no desire or incentive to compete.

  • We will perform visits on a quarterly basis, or as needed after significant events.

  • We will give medication and care considerations, not directly prescribe or order, unless the PCP requests it.
