Consultation Includes:

1. Medical Issues

We'll review your medical challenges through the lens of an expert. 

The more information, the better. 

Provide as much as you can.

We also have access to many records and hospital systems.

4. Care Preferences

Some people want only comfort care.

Others agree to treat things that are 'easy to fix', but going to doctors and hospitals can be difficult or stressful.  Some want to do everything they can.

2. Medications

We'll make sure each medication serves a purpose.  It's okay if you're not sure, but fill in as much as you can.  

If there is another list (such as recent discharge from hospital, go ahead and include that, and we'll help figure out the 'best' list for your situation.

5. Confirmation of Info

Once Medical Records, Medications, Quality of Life form, and Care Preference forms are filled, we'll call to clarify any incomplete or unclear information.

3. Quality of Life

This is a survey that will give us a sense of who you are and what matters to you.  If filling out for a loved one, fill out to the best of your knowledge what you feel that person would say -at this moment in time- if they could properly speak for themselves

6. Consult Visit

After thorough review and analysis of the information and forms, the physician will perform the consultation visit, take a deep dive into the results and meaning, and come up with a comprehensive action plan.  Afterwards, a summary will be sent to the patient/family and primary care provider.
